You have to enter the code from your winning game piece along with your personal details.

After submitting your claim, you will be contacted by a promoter, who will instruct you to continue your claim. You will be sent paper forms to fill and return in a pre-paid reply envelope. You should not send in your winning pieces in the envelope.
